• Code 98975 is eligible to be billed 16 days after the patient activates their account.
  
  For example:
• Day 1 - The patient is set up on the app
• Day 2 - The patient activates their account (achieves 1 data point)
• Day 17 - 16 days after successful account activation, 98975 will be listed as ‘Ready for Billing’.

Frequency:

• 1x per episode of care

Average Reimbursement: ~$20

Notes:

• Code 98975 can be billed again on a new plan of care for patients that were discharged and then readmitted.
