What is a Daily Outcome Score?

The Daily Outcome Score feature let's patients rate how they're feeling each day, helping gather important data in real-time.

This feature will be a huge help in boosting patient's app activity and easily meet the criteria for RTM CPT code 98977 requirement.

Regular tracking of a patient's well-being assists providers in modifying treatments as needed, ensuring better and personalized care. 

Patient Mobile App:

• This pop-up will ask patients for a daily outcome rating per day.
• The Daily Outcome Score appears when the patient first logs into the app each day.
• Once they have logged in, the patient will be prompted to indicate how they feel by clicking on the emoji that best describes their emotions on that day.

Mobile App: 

• Once they have logged their outcome score, a message will pop up notifying them that a Daily Outcome Score for the day has been submitted.
  
  
• Daily Outcome Score submissions will be visible on the Patient RTM Details Tray.
  
  

• Providers will also see the scores on the patient's Adherence log in PT Wired.

• We've added a Daily Outcome Score threshold email notification option for low scores. Providers can customize the threshold and enable/disable notifications as desired.

Notes:

• Patient's can disable the feature via the app settings if they prefer.
• The Daily Outcome Score is now added as an RTM Data Point and is now included in RTM code 98977 RTM Documentation.
